- Summary of this article：In 2,215 AFIRE participants with atrial fibrillation and stable coronary disease, we compared those with versus without rhythm-control therapy. Rhythm-control patients were younger and lower risk. Unadjusted outcomes favored rhythm control, but after multivariable adjustment and propensity matching, the composite of stroke, embolism, myocardial infarction, unstable angina needing revascularization, or death did not differ (adjusted HR ~0.75; p≥0.30). Thus, the apparent benefit reflected baseline differences, not therapy, under rivaroxaban ± antiplatelet care.
- 論文の要約：安定した冠動脈疾患のある心房細動患者2215人を解析し、リズム治療（不整脈を元のリズムに戻す治療）の有無で成績を比べました。リズム治療群は若く低リスクで、未調整では脳卒中などの複合転帰が少なく見えましたが、背景差を調整すると有意差は消えました。リバーロキサバン単独・併用下では、独立した上乗せ効果は確認できませんでした。
