Summary of this article：This retrospective observational study aimed to identify the factors
contributing to unpaid
medical charges among uninsured, non-resident foreign patients hospitalized at a tertiary care facility
in Tokyo.
Utilizing medical and administrative data from 153 eligible patients admitted between January 2023 and
February 2025
(excluding elective medical tourism), the analysis found that 9 patients (5.9%) had outstanding hospital
bills upon
discharge. Multivariate logistic regression analysis identified the absence of third-party financial
support (primarily
travel insurance) as the main independent predictor of nonpayment (adjusted OR: 0.12; p=0.040). The
total amount of
billing was also found to be statistically significant as a risk factor (adjusted OR: 1.01; p=0.039).
These exploratory
findings highlight the importance of private insurance in mitigating financial risk for hospitals.
