神経・筋疾患治療研究グループ
進行性の難治性神経・筋疾患に対し、なんとか有効な治療法を開発することは、研究を行う神経内科医の重要な使命です。当研究グループでは、筋ジストロフィーのひとつである筋強直性ジストロフィーの治療法開発を中心に研究を進めています。また、遺伝子上の塩基繰り返し配列が異常に伸長しておこるリピート病(ハンチントン病など)についても研究を行っているほか、核酸医薬による遺伝性パーキンソン病の治療研究も進めております。
研究紹介
筋強直性ジストロフィー症の病態解明と治療法開発
筋強直性ジストロフィーは、成人で最も多い筋疾患で、 筋強直(握った手を離せない、など)や進行性筋萎縮のほか、 不整脈や内分泌障害など多彩な全身症状を呈します。 この病気では、遺伝子上のCTG繰り返し配列が異常に伸びており、 そこから転写された異常RNAが選択的スプライシング機構を障害して種々の症状を引き起こします。 我々のグループは、これまでにリアノジン受容体や筋小胞体カルシウムポンプ、 筋細胞骨格蛋白など続々と本症でのスプライシング異常を発見しており、さらなる病態解明へ向けて研究をすすめています。 また本症では異常RNAをターゲットとした治療研究も進められており、 我々も核酸医薬や低分子化合物によるモデル動物での治療効果を数多く報告しているほか、現在医師主導治験も実施しています。 国内はもとより米国・フランス・ポーランドなどのグループと緊密に連携をはかりつつ、一日も早い治療法の確立を目指しています。
リピート病の病態解明と治療研究
遺伝子上の塩基繰り返し配列(リピート)の異常な伸長が原因でおこるリピート病は、 筋強直性ジストロフィーのほかにも、CAGリピートの異常伸長によるハンチントン病・遺伝性脊髄小脳失調症や、 CGGリピートによる脆弱性X症候群、GGGGCCリピートによるALSなど様々な疾患があります。 これらのほとんどは根本的治療法のない難治性疾病であり、多くの方々が苦しんでおられます。 我々はリピート病共通の原因であるリピートの伸長機構を解明し、 その制御による治療法の開発を目標に、大阪大学産業科学研究所や米国・カナダの研究グループと協力して研究をすすめています。 最近では、CAGリピートに結合する核酸標的低分子により異常伸長リピートを短縮できることをハンチントン病モデル動物で実証しており、 今後リピート病全般への治療応用が期待されています。
核酸医薬による遺伝性パーキンソン病の治療研究
遺伝性パーキンソン病のひとつPARK4は、過剰なαシヌクレインが神経変性を引き起こします。 日本医療研究開発機構(AMED)の研究事業として、神経変性グループと共同で、 αシヌクレインを抑制する核酸医薬を用いたPARK4の治療研究も行っております。 将来的には、孤発性パーキンソン病やレビー小体型認知症などαシヌクレイノパチーの 根本的治療薬としての展開も目指しています。
